Assertive Community Treatment (ACT) Team Clinical Case Manager

We have a full-time position available on our Assertive Community Treatment team as a Clinical Case Manager. This dynamic multidisciplinary treatment team is dedicated to delivering intensive community-based support to severely and persistently mentally ill (SPMI) adult clients served by Greater Nashua Mental Health Center.

This position features a 4-day work week through a rotating tri-monthly schedule.  The ACT program offers a competitive salary due to the “wrap-around” service expectations of staff.

The ideal candidate will be a highly motivated and flexible problem-solver who can provide the increased level of care expected in the ACT program. A natural passion for helping others is a must.

Duties include case management, home visits, coordinating supports in the community, crisis intervention, and assisting clients with daily living skills.

Candidates must possess a Master’s degree in Psychology, Social Work, Human Services, or Rehabilitative Services.  At least 2 years of experience working closely with mentally ill adult clients is preferred. Training and experience with Evidenced Based Practices and Illness Management and Recovery (IMR) is preferred. Fluency in Spanish is a plus!
